Bombardier CL600-2B19 CRJ, Hilo, Hawaii, Feb. 13, 2008–The 53-year-old captain and the 23-year-old copilot fell asleep on the Go! Airlines (a subsidiary of Mesa Airlines) flight from Honolulu to Hilo and flew 26 nm beyond General Lyman Field, the flight’s destination. Fatigue was a suspected factor, as the captain had flown four legs in eight hours on duty on the 10th, eight flights in nine hours on the 11th and eight flights in nine hours on the 12th. The first officer was off duty on the 10th; on the 11th and 12th, he had the same schedule as the captain. Adding to the stress, they said, was the fact that previous flights had had a partially functioning flight management system, compounding their workload, and on the morning of the incident a flight attendant was 30 minutes late reporting for duty.
About halfway through the February 13 flight, after acknowledging the ATC clearance to paris intersection and turning toward it, the pilots of the CRJ, with 40 passengers aboard, fell asleep and failed to reply to ATC calls. After flying past the destination airport, the crew called in and returned to the airport.
